I would like a UK PHP developer to create a PDF test scenario.

1. Create a Form that the user may enter certain information such as Name / Address.

2. Create a file upload button - the user may upload a JPEG / PNG image that will be placed on the PDF. Resizing may be needed also.

3. Form is submitted the code will then produce a PDF that will display the entered information. The layout will be structured for this test scenario so nothing too complicated needed.

4. The complication is that I would like the test to also include the ability to show on the PDF Chinese / Japanese / Korean and any other Multi-Byte language. This is crucial. So the user would enter these Kanji through the form and then it would show on the PDF, optional save into MySQL DB also.

5. You may use PDFLib, FPDF, or any other library that can create the PDFs dynamically through PHP.
