Simple Corporate Website

We are a small medical products supplier, and at the present have a single holding page for our website. We would like build a small, but high quality corporate website. We will supply all text and photos for the content.
It will feature the following pages, sections.
About Us
- History of the Company
- Management
- News
Products
- 6 templated product pages
Support
This area will need to feature a helpdesk system of some kind.
Contact
Featuring a form that will collect key information and then send it via email.
General Requirements:
- HTML for pages.
- No flash
- PHP / MySQL for advanced items
Please only respond if you meet ALL of the following, these are non negotiable:
- Must have a current IC9200 certification, or at least an application number. ( http://www.icharter.org/certification/ic9200/ )
- Must be able to supply end results in Macromedia Dreamweaver format ( http://www.macromedia.com/software/dreamweaver/)
- Must be able to provide on going support (at a fee to be negotiated).
