PDF form script (CGI?)

We have created a PDF form which will be put up on a website. The user will fill in the form and click Submit, whereupon the data will be sent to the server.
We would like then to have a PDF re-created on the server that is the same as the form, which the client can then print out. I understand that the data has to be sent as FDF format, and that I need a CGI? script to process the data.
Please supply costs and timescales for creating this working script, and any examples of PDF forms you've been involved with before.
