Creative required

We need a creative for a web project we've got under way. We need a PSD file which we'll turn into HTML ourselves, plus logo design.
The page creative would need to be a single page mockup. The final deliverable would be a single PSD, layered, with components grouped into layer sets for us to extract graphics easily.
Please respond with 3 examples of your work and a price for initial concepts plus refinement.
