We built something like that for a medical charity... the test site's still
there, I think, at http://www.datasmith.co.uk/sar/. We did it for free, and
you're welcome to the code for free if you want it, provided you give us
some credit.

It uses an access database to build the data from multiple and repeatable
forms. Passwords, but no registration (although that would be easy to add).

Feel free to try it... this is just an old test-site, there's no real user
on it. Let me know if you want the original sources.
