I usually use NetBanx:

No monthly fee. One off setup fee £500 for a bespoke integration. Cheaper
if you're using one of the supported packages [EMAIL REMOVED] is £150 I
think). Transaction fee of 4% of sales on a sliding scale down to 1%
depending on turnover.
