Confidentiality Statement

I'm just finalising quite a large proposal which I have to get to the
printers tomorrow to be bound and I need to finish it off with a
confidentiality statement which just basically says something along the
lines of "All information in this document is confidential and copyright to
xxxx blah blah blah... reuse without permission etc etc etc..."

Does anyone have a standard one of these which I could use?

This is quite important, so if you could send it off list that would be much
appreciated.
