sounds like you could probably get by with a simple form to mail script.
they are very common and often provided as a sample script by your hosting
company. These will also come with reasonably clear instructions, check
there first. If not, to find one that suits you, just use your favorite
search engine and type in "form to mail script", you will find any number of
free and configurable scripts with instructions, or serch for "form to mail
tutorial" with your favorite search engine and pick a tutorial that you find
freindly and roll your own.
