Hi Debi

The easiest one to use is Form to Mail which you can download from
http://www.shoepalace.com/scripts/

You just edit the script to the instructions which is just putting your
smtp server, referrer and upload the script to your cgi-bin folder,
which you might not get with geocities but have a look on their website,
they might have an alternative or ready installed script. You link to
the script via your form in Dreamweaver using the Get Post method.

Read the instructions and hep on the website first.
