FN-FORUM: lingo question

I have a CD Rom project to do for my dads shop originally he wanted a
website now hes decided on a CD business card which he can hand put to his
customers. He wants to put a questionare form on the CD Rom so that he can
get client feedback, all they would have to do if they have the internet is
log online - hit the send button on the form and then send it straight to
his email address.
Does anyone know how to do this lingo in Macromedia Director.
