Sometimes Dreamweaver thinks the file you're uploading is older than the one
on the server and decides to ignore the upload thinking you don't want to
overwrite a newer file. And sometimes it asks. Damn annoying - just delete
the server version and do another upload.
I had a similar problem with the zero byte files - as far as I could tell,
the server thought that the file was in use by a site visitor and so an FTP
file lock was in place meaning I couldn't replace the file but ended up
creating zero byte files when I tried - it happened with everything from
small HTML files to PDFs. I've only ever experienced this on one ISP, so it
might be the way they have got their systems set up. I had to wait until
after hours to upload the files, or just keep bashing away when an update
was urgent, slowly working through the list of files (I didn't have the
ability to "switch off" the site to be able to do an update unfortunately).
