Basically, there is no fool-proof method.
People using AOL in particular will get a new IP address every time they
sneeze.

By busy I mean lots of users - so they may come from same workplace, have
same Isp etc.
I could go with the cookie option too - not foolproof of course but another
layer...!

Why would it not work for busy sites? Once someone has voted just store the
IP in a database and you could also set a cookie on that persons machine as
a second check.
