FN-FORUM: HTML / PHP Webform software recommendations please

Hi Guys.

Not sure if I asked this a couple of days ago (goldfish syndrome setting
in I think). Would any of you kind folk care to through me some fish
food and get rid of that damned cat.

But that aside, I'm getting bored with manually building forms for
websites. There must be a free or inexpensive GUI / WYSIWYG tool for
building forms, that can then be easily integrated into a PHP
header/body/footer type site.

Any recommendations pleaseeeeee.
