Server-side:
MailScanner, using spamassassin, SARE rules, ClamAV with sanesecurity
rules. Stops pretty much all spam at the server, quarantines all messages
to protect against false positives....

I'd go with an external provider such as email systems or black spider -
you can resell to your client.
I get next to no spam - maybe one a day.
