Jon,
I bet he uses the phrase "Can you just do the following..."

There is a simple solution. Price yourself to a level you can accept
working with him (short deadlines means you are working double time into
the night etc). Request 50% payment up front (poor payment history) and
long lead times due to other work commitments (mitigating the deadline
issue). You don't have to say goodbye and it is better for him to choose
to find somebody else rather than telling him to f*ck off. Don't burn
bridges. Seriously just don't. Last thing you need is him bad mouthing
you to the local business community.
"Hey Jon can you do X by next week?"
"Sorry I'm fully loaded for the next four weeks, but can fit you in
then." OR
"Yes I can, but you haven't given me enough notice to get it done on top
of what I already have to do, so will be doing it outside of normal
hours at double time. "
Each one followed by:
"I'll need a retainer of X because of your past payment history with
me." (70% of estimated time on your part at least :) )
He will either go somewhere else or actually start working with you in
the way you want. Just don't burn bridges. Let him make the choice. To
be honest he sound more likely to run and find somebody else to abuse.
