Try doing a print_r($_REQUEST) at the top of the script to see whats
been passed over. You should also add enctype='multipart/form-data'

It depends how you pass the variables from the form (method GET or POST)
Anyway you can track what is passed using a simple var_dump in the
script that's supposed to process the form...
