Guessing games are great fun but I can't help thinking that this would all
be over in about two minutes if you would just post two things:
- the HTML of the page as rendered (i.e. from view source)
- the output of print_r($_POST) (where this is the *first* line in the file).

Or stick them on the web somewhere, rather than cluttering the list
with great screeds of code.
